Description:Sholom Community Alliance, one of the leading senior housing and healthcare service organizations in the Twin Cities, is seeking an on-call COTA to join our team at the Shaller Campus in St. Paul. This individual will provide direct & indirect Occupational Therapy services in compliance with AOTA guidelines, professional code of ethics and Sholom Comunity Alliance policies and procedures. The nature and scope of services provided must be in accordance with state and federal regulations.
Duties:Responsibilities will include:
  • Assessment: The COTA will obtain pertinent data necessary for identification or treatment of those in need for skilled intervention from medical records, patient observation and interview and/or the administration of standardized tests in accordance with physican orders. The COTA may contribute to the assessment & reassessment process under the supervision of an OTR.
  • Treatment: The COTA while under the direction of the OTR will plan, direct and conduct programs to develop, improve and/or restore the performance of necessary functions, compensate for dysfunction and/or minimize debilitation.
  • Consulation: The COTA will provide explanatory instruction to the patient, primary caregivers and medical personnel to help them understand and deal more effectively with patients. This will include the attendance to patient care conferences and any other interdisciplinary team meeting.
  • Documentation: The COTA will keep timely treatment records, progress notes, patient files and written plans of care as required. The COTA may contribute to the formation of the intervention plan under the supervision of the OTR.
  • Supervision: The COTA will receive direction & supervision from the OTR as defined fy official AOTA documents. The amount of supervision must be provided in accordance with state law & regulation.
Qualifications:
  • Graduate of an accredited entry level occupational therapist assistant program.
  • State credentialing/licensure as required.
  • Experience in LTC & membership in professional organization is preferred, but not required.
  • Sound clinical judgement relating to the provision of OT services.
  • Collaborate with members of the interdisciplinary team in delivery of patient care to maximize functional outcomes.
  • Knowledge of federal & state regulations & professional guidelines related to the delivery of OT services.
  • Ability to relate positively, effectively and appropriately with nursing, social services, residents, families, community members, volunteer and other facility staff.
  • Meet all health requirements as required by law.
  • Ablility to read, write, and speak and understand English.
