| Description: | To evaluate, initiate, document and delegate a resident’s occupational therapy program. To direct the operation of the occupational therapy department within state and federal regulations

| Duties: | EXAMPLES OF WORK (A position may not include all the work examples given, nor does the list include all that may be assigned)
- Plans and implements resident evaluation and treatment programs in accordance with physician’s orders.
- Direct resident care and safety while the resident is in occupational therapy department and or involved in occupational therapy procedures.
- Supervise work of other personnel specifically trained in restorative and/or maintenance services; e.g. Rehab Aide, Nursing Assistants, COTA.
- Responsible for documentation of services provided which meet federal and state regulations and the medical practices established for Sholom Community Alliance.
- Responsible for communicating resident’s progress to physicians, family and other health disciplines.
- Participate in in-service training for rehabilitation staff and other home personnel.
- Supplies Director of Rehabilitation with statistics for charges and monthly/yearly reports.
- Develops appropriate communication tools for staff to ensure best quality care for client/tenant.
- Participate in planning, constructing and implementing policies and procedures.
- Works as a team player.
- Attends meetings as needed.
- Review equipment needs periodically with director of Rehab.
- Promotes a positive, team approach and a healthy work environment.
- Is available on-call
- Assures compliance with regulations and is available for federal and state licensure surveys.

| Qualifications: | MINIMUM REQUIREMENTS TO BE ELIGIBLE FOR HIRE
- Graduate of accredited school of Occupational Therapy
- Must be registered and licensed with the State of Minnesota as a Occupational Therapist, and must maintain licensure.
- Must have a valid driver’s license
- Must pass a criminal background check.
- Must be able to safely perform the essential physical tasks.
- Must be fluent in English (some positions may require fluency in other languages as well).

- To perform this job successfully, the individual must be able to perform each essential duty satisfactorily.
- Must be familiar with general nursing and geriatric nursing practices.
- Must demonstrate leadership, good judgment and dependability.

- Must be able to perform duties of unlicensed staff.
- Must possess a desire to work with aged, infirm and handicapped persons.
- Good organizational and planning skills.
- Must be able to train and teach procedures, policies, etc.
